Lancaster, SC Lancaster, SC Part-time Full-time PRN Per diem Part-time Full-time PRN Per diem From $68,000 a year From $68,000 a year 3 days ago 3 days ago 3 days ago Providence Care is inviting Registered Nurses to join our growing Home Health or Hospice teams in the Lancaster, SC and surrounding areas by providing full time, part time or PRN (Per Diem) Case Management services to patients in their homes, assisted living and long term care facilities who are enrolled in our home health or hospice programs.

Responsibilities:
Provide patients and families with the care and compassion they deserve at this time in their lives.
Coordinate care with a team of professionals, including medical clinical director, medical social workers and other passionate caregivers.
Collaborate with Physician for new orders, to advise of needs and/or changes.
Provide education to patients and families Attends and actively participates in weekly team meetings Provides rehabilitation and coordinates medical needs for clients Medication Monitoring and Administration by oral medication or needle injections Orders, reviews, monitors, and discontinues medications as ordered by the physician Conducts assessments and approves plan of care and service delivery methods Performs competency and skills evaluations on employees who provide direct care Informs the physician and staff of changes in the client's condition Writes progress notes, summaries, and discharges Implements the treatment/rehabilitative activities and strategies Reports significant/major incidents or accidents and notifies proper authorities of circumstances or situations that present possible health hazards Follows consumer health plans and provides and promotes consumer choice and facilitate independence Assists in maintaining and updating nursing, pharmacy, infection control, and health and safety policies and procedures Other tasks as assigned
Qualifications:
Must have a RN license eligible in the state you are applying Willingness to be flexible and work some on-call hours potentially as needed Reliable transportation and valid drivers license Passion for helping others Wound care experience is a plus Willingness to travel and attend to patients in their home setting Learn more at:
